Before your first visit, we will need you to download, print, and fill out new patient registration forms. By filling these out at home, and bringing them with you to your first visit, you will be able to save time in the office filling out paper work.
Please note that if you have gone 3 years or more since your last visit to Savannah ObGyn, you are considered a new patient, and will need to fill out patient registration forms.
Accepted Insurance
Savannah ObGyn is part of the CIGNA Care Designation program, which enables us to offer discounts to patients covered under CIGNA.
We also accept Aetna, Ambetter, Blue Cross/Blue Shield, Care Network, Humana, Kaiser Permanente, TriCare Select (and Prime on referral), and United Healthcare. If your plan is not listed, we may still be providers. Please contact your insurance company’s member services to confirm your in-network coverage.
We have privileges at St. Joseph’s/Candler and are only accepting new patients who can receive services St. Joseph’s/Candler. All surgeries and deliveries will be performed at Candler Hospital.
*Please note that it is the responsibility of the patient to make sure that our providers are in network.
Medical Records Release
If you would like to have your medical records released to other providers please fill out the Medical Records Release FROM SavOBGYN form. This form gives your consent for Savannah ObGyn, P.C. to share your personal information and records with a specified physician.
